Clearance Requirements Checklist
Results of ALL clearances, as indicated on the form, must be submitted to your Department Office. Please do not submit until you have ALL four clearances. Please bring originals and one copy to turn in at the time you submit this form. Please keep your originals in a safe and accessible location during various requirements of the University and K-12 school district. If your department is not listed below, return this form to Miller 111.
Students not complying with this requirement will NOT be permitted to participate in clearance required courses.
