Please arrive at the Ghering Health and Wellness Center about 15-20 minutes before your scheduled appointment time to complete the necessary forms to receive services. Stop by the reception window in the waiting room when you arrive to receive these materials. They will ask for your demographic information and consent to services.
To save time when you arrive, CAPS recommends that you review the Informed Consent to Services Statement now in order to avoid being delayed in meeting your psychologist as scheduled. A copy of this consent form will be provided to you, upon your request.
